I am very confused about condom sizes. How literally sized are "small", "regular", and "large"?

Different brands have somewhat different sizes, but it seems that if you have a girth of 5 1/2 inches or more a large size may be more comfortable. Note that length doesn't make a difference - all condoms are made to be considerably longer than the average male and you just unroll as far as you need. The large condoms are barely longer than the regular (which leaves unusually long guys in some difficulty).
I've never seen a small condom labelled 'small' - it's usually 'slim' or 'snug' or 'better-fitting'.
Trial seems the best approach.

Trial is difficult cos I always start to get incredibly unaroused by messing around with condoms while trying to maintain an erection to some porn, but I will keep trying.
How tight should it feel? Also what should I do if I partially lose my erection, causing the condom to become slightly looser and thus becomming creased and folded even once a full erection is regained (this is a major problem during experimenting)
I have these condoms that say "54mm width". That seems like a really weird size indicator to use, would be a hell lot easier to understand if they used "girth" or "circumferance". Am I supposed to look at its "width" in terms of a top down approach or a side on approach :s
Is it normal to find the head uncomfortable but not the rest of it? I have sometimes thought from looking at porn that perhaps the head of my penis is proportionally larger than normal in relation to the shaft. This can hurt when putting on a condom and while its on the head of my penis looks constrained and smaller.
My girth is somewhat over 5" which I gather is more or less average and I would have thought these regular sized condoms would do the trick, but once a condom is one I am never sure whether this is how its supposed to be or not.

They should cover the skin smoothly but not be uncomfortable. I've heard of a version that has more head-room - that's probably what you want.
